• PhD Academic Registration Forms and Information - A variety of information of interest to doctoral learners related to academic registration and standards of academic progress.
• Access diploma order forms for Union - Diploma orders are placed automatically by the Registrar's Office for all graduates of the university at no additional cost.  The diploma will reflect the degree title, and the learner name as listed in the university Student Information System database.  The first diploma will be mailed to the graduate at the address on file at the time it is released.  Additional diplomas may be ordered through the Registrar's Office
• Access the Transcript Request form - the learner's first official and learner copy of the graduation transcript will be released automatically by the Registrar after notification by the Dean of the graduation recommendation and subsequent review.  Additional official transcripts may be ordered using the transcript request form.
